This is the Wood Village Home-Based Business License Form. Along with this form, a Business License Application and an annual fee of $50, which may be pro-rated, are required for submittal to the City of Wood Village. You may pay the entire fee in person at the Wood Village City Hall, or pay it online after you submit this form. If your business makes less than $25,000 in gross revenue, you are exempt from the $50 fee. However, you will need to send in a copy of your Schedule C for verification and qualification purposes.
